Register as a patient
We welcome new patients who wish to register with us.
To register with our surgery you will need to complete a registration form and a health questionnaire. The forms are available to collect from the surgery during our opening hours or alternatively please use the link below to submit your registration online.
Once you have completed the registration form either in person or online you will receive a letter from the practice confirming your registration and providing details of your named GP at Esk Valley Medical Practice. We would ask that once you have received this letter you contact the practice to arrange an appointment with the health care assistant for a new patient health check.
